Medical Content: Outpatient medical (will not include major surgical procedures)
Team Leader: Gloria Halverson, MD, who has been involved in five previous trips to the House of Hope and is actively involved in human trafficking work. She is a member of the CMDA Trafficking in Persons Task Force.
Lodging: a local hotel where we will also take most of our meals and have times of fellowship together. There are 3-4 people per room. There are no separate rooms for married couples

Complete this form giving GHO permission to book your travel and set up payment information for your trip.
Waiver of Responsibility Required
Form acknowledging a waiver of responsibility and assumption of risk. Must be completed by each participant
Minor Supplement Application
Supplemental application for minor children. Must be completed for applicants 17 years or younger.
Parental Consent Form
Form to be completed if a minor is attending a GHO with one parent. The Minor Supplement Application must also be completed.
Temporary Legal Custody
Form to be completed if a minor is attending a GHO trip without an accompanying parent. The Minor Supplement Application must also be completed.
Cancellation Form
Form to cancel from trip. Application fee of $150 is non-refundable. Form must be submitted to be officially cancelled from trip.

Prerequisites
- Important Note! The expiration date on your valid passport MUST be 6 months beyond the return date of your trip. It may take 3 to 6 months to receive a passport from your local passport office; therefore, you may consider obtaining your passport via g3visas at www.g3visas.com.
- Must have experience serving people
- Some foreign language skill is greatly appreciated
- Vaccinations
Responsibilities
- Attending morning and evening team meetings, including devotions
- Serving in the capacity of position on team and/or at direction of the team leader
Restrictions
- You must be 18 years of age to join this trip
- College students are welcome
- Most team members must have some medical experience but there are opportunities for some people with no experience
